Cerebrospinal fluid (CSF) leaks are diagnosed through a combination of clinical evaluation and specialized imaging tests. The process typically begins with a thorough medical history and physical examination, focusing on symptoms such as clear nasal discharge or headaches.

Imaging Techniques:

  • MRI with Gadolinium: This is often the first imaging test used to detect leaks in the brain, providing detailed images of the CSF pathways.
  • CT Cisternography: Considered the gold standard for cranial leaks, this test uses contrast dye to pinpoint the exact location of the leak.
  • Myelography: This involves injecting contrast dye into the spinal canal followed by CT imaging to identify spinal leaks.

Additional Tests:

  • Beta-2 Transferrin Testing: This laboratory test is highly specific for confirming CSF leaks, as beta-2 transferrin is found only in CSF and perilymph.
  • Lumbar Puncture: Sometimes performed to measure CSF pressure and analyze its composition, though not always necessary for diagnosis.

These diagnostic methods work together to confirm the presence and location of a CSF leak, guiding appropriate treatment options.
